My password manager autofill put the wrong credentials into my bank login

Last Tuesday in Austin it dumped my old email and password into the wrong field, locked me out for 20 minutes, and now I double check every single autofill before hitting enter, has anyone else caught theirs doing something weird lately?

Mine autofills my ex's info sometimes, guess that's one way to relive the past.
