Had to pick between leveling a slab with a $50 bag of self-leveler or re-pouring the whole thing.

Yeah Shane nailed it with the base prep thing. I had a similar experience where I thought I could just pour self-leveler over an old slab without really cleaning it right. Ended up with cracks all over within a month. The funny thing is the self-leveler itself was fine, but the bond was garbage because I didn't etch the surface or use primer. Now I always spend the extra time grinding and vacuuming before I even open the bag. It's a pain but beats having to chip out a failed pour.
