Almost lost a customer over a clogged cap on a rental property

Last month I was doing a routine sweep on a three-story rental in Providence when the vac hose started pulling almost nothing. I checked the flue, the damper, even the cleanout door. Took me 40 minutes to realize the previous crew left a plastic cap wedged in the tee, right where the liner meets the smoke chamber. The tenant said the fireplace had been smoking all winter. I popped it out with a fish tape and a coat hanger, ran the brush again, and the draft came back strong. The landlord was ready to blame me for the whole mess, so I took a photo of the cap and sent it to him. Has anyone else found leftover debris from a prior sweep that should have been caught during their own final check?
