Hit 500 stair stringers and just now figured out my layout mistake

I've been framing stairs for about 15 years, mostly residential work outside of Columbus. Last month I finished my 500th set of stringers, which felt like a big round number. I was going through my old notebooks and saw a pattern, I was cutting the first riser height wrong on almost every one. Not off by much, maybe a quarter inch, but it stacked up on the top step. I fixed my layout method on the last 20 jobs and the fit is noticeably better. Has anyone else hit a milestone and found a basic flaw they'd been repeating for years?
