My case worker said I was using the wrong form for SDI extensions

Was filing for my SDI extension like I always do and my case worker calls me back. Said I been using the old DE 2501 form for over a year and they changed it last March. She sent me the right one and it got approved in like 5 days instead of 3 weeks. Anyone else been using outdated forms without knowing?

It's on you to keep up with the latest forms, not on them to hand-deliver updates.
