Honestly, I welded a 4 inch schedule 80 pipe with a 6010 rod in a tight spot last month and the bead looked perfect, but it failed hydro in under a minute.

Man, that's brutal. Could've been a tiny slag trap you couldn't even see in that spot.

Slag doesn't really work like that.

Wait, what do you mean it doesn't work like that? Slag can totally hide in a seam and then pop out later. It's not always a big, obvious chunk. A little piece gets trapped during the pour, you clean up the surface, and it looks fine. Then it works loose after some heat cycles or vibration. Seen it happen.
