Hit a rooftop gig in Pittsburgh and found a hidden hazard

I was up on a 12-story building last month doing a boiler stack repair and noticed the parapet coping was loose in spots. Not a big deal on paper, but with the wind up there, one gust sent a chunk of concrete past my head. The GC never flagged it in the JSA, and I've seen this on at least 3 other jobs in the last two years. How many of you check the whole roof edge before you start torching, or do you just trust the site survey?

Always check the edges myself... trust nothing.
