Yeah you nailed it. The show does that thing where they trust you to remember stuff from earlier and put it together yourself. Rewatching is the only way to catch half of what they're doing honestly. Go back and watch his scenes again with that kid in mind. You'll see him glancing toward the waiting room doors every time a nurse comes in. His whole rhythm is off from minute one. It's like when you're cleaning a house and you spot one thing out of place and it messes up your groove for the rest of the job except way more intense obviously. That little background detail changes how you read everything he does after.

Oh man, I used to think that scene came out of nowhere too. I was sitting there like "wait, what just happened?" but then I rewatched the episode and caught that kid in the background of the opening shot. That's the thing about good writing, they don't just throw in random details for no reason. I feel dumb for not connecting it the first time because it's so obvious now that you point it out. It makes the whole breakdown hit way harder when you realize that dude was carrying that weight the whole time.
