That time my AI model hallucinated a fake lawsuit and I almost lost a client

I was demoing a chatbot for a legal firm in Austin and it invented a whole case citation out of thin air - judge, docket number, everything. Had to scrap the whole presentation and spend 2 days retraining the RAG pipeline with stricter confidence thresholds. Anyone else have a production AI moment that nearly backfired?

Honestly, that's my nightmare. 35 minutes into a client demo and my chatbot cited a patent that didn't exist.
